West River Health Services is offering well child exams with athletic physicals for the upcoming school year.
Date: June 19 and June 26
Time: 9:00 a.m - 4:00 p.m.
Well child exams and athletic physicals will be performed by WRHS physicians and colleagues. Immunizations, medication refills, screening labs, management of acute or chronic conditions, and the athletic physical will be completed during this time. Most insurance companies allow one well child visit at no charge per year. Wellness exams will go through your insurance and sports physical forms will be filled out for free if completed during wellness exams.
An annual sports physical exam is required for all athletes participating in sports grades 6-12 in North Dakota schools. Sports Physicals for athletes in grades 4 & 5 are suggested but not required. Please have all the appropriate forms filled out and signed by parent/legal guardian and student prior to coming. We will not be able do the exam on the student if these forms are not completed and signed.

If you are selecting the athletic physical only, please bring $25.00 with you at the time of the physical. The reduced fee of $25.00 ($40 regular price) will only be honored on the above designated dates. If your child is unable to attend either designated exam dates, well child exam or athletic physical only appointments can be scheduled at any of the West River Health Services Clinics.
Please have your child wear shorts and a t-shirt or tank top to the visit. If the medical provider finds a problem, he or she might recommend certain tests and a follow-up exam by your primary care physician or specialist before the student is cleared for participation.
